The following are screenshots of a foggy, medieval port city I made for a recent game project using Unreal 4.  These screenshots were taken in the Unreal Editor. 

I placed all of the buildings and props, created many of the PBR materials, sculpted and painted terrain as necessary (in this case mainly streets, ground, and ceilings in underground areas), and I set up all lighting and skies.














Later in the game, players return to this city while it is burning...



There is also an underground section of the city...
